Application
for certification of film for exhibition
Any
person who desires a film to be certified as suitable for public exhibition shall
submit an application, in writing, to that effect. Every such application shall
be made in form as follows : FORM

The Secretary Bangladesh Film Censor Board Government of the People's
Republic of Bangladesh Dhaka. Dear Sir, I
apply for the certification for public exhibition of the film mention below. A
copy of the press book of the film (in respect of imported films) three copies
of synopsis and full text of songs (in respect of locally produced or vernacular
films); import documents (in respect of imported films) and dialogue script shooting
script are enclosed. The examination fee of Taka reckoned at the rates prescribed
in the Bangladesh-Censorship of Films Rule 1972, has been deposited in the Treasury
under the Receipt Head "Code No. 1-3371-0000-2681" as per challan attached.
2. I undertake
to be governed by the Censorship of Films Act, 1963 (Act XVII of 1963), and the
rules made there under. 3.
Necessary particulars as required by the rule 14 of the Bangladesh Censorship
of Rules, 1972, are given as under: - (a).

Is the film in its original version as produced or imported or dubbed? If dubbed,
